I keep getting comment indicatrs on this thread, hopefully this will fix your problems: I reset the dam to its default state, completely exited out of the game, restarted, upon last checkpoint I straight lined to the grenade, which was there, threw it in the water, set the dam to the state used to progress, then the guy was where was supposed to be and I was able to do the quest and receive the achievement. So, I went back to where the grenade is located, picked it up for the 2nd time, and attempted to throw it in the designated area where the "Throw Grenade" action appears.
That doesn't work. The bug is that you simply cannot throw it, not that you're just in the wrong place. Let me know if that worked for you. Problem is, holding down A or the primary mouse button results in nothing; I am assuming this is some sort of bug because I already threw the grenade on the same game, but it did not save it even though I received about 3~ "Saving Scene" indicators afterwards yesterday. Would be nice if the devs would chime in. In the forest near the space ship, if you go as far north as you can, until you reach a vista, you'll find a grenade next to a rock. I tried to re-activate the dam so I can throw nade and everything but nop. Afterwards, head back to the dam control room and go through the building to the other side where the dead end was. Problem is, holding down A or the primary mouse button results in nothing; I am assuming this is some sort of bug because I already threw the grenade on the same game, but it did not save it even though I received about 3~ "Saving Scene" indicators afterwards yesterday. Take it to the stone bridge before the portal house and after the spaceship, you'll get a prompt to toss it in the water from one of those small nooks across from the bench side.
